Targeted Case Manager (KY MOMS MATR)

Salary - $35,000
Educational Requirements: Targeted Case Manager must possess a bachelor's degree in behavioral science or be a certified alcohol and drug counselor who has a bachelor's degree in behavioral science. Must have at least one (1) year of full-time employment experience working directly with adults in a human service setting.
Duties/Responsibilities: Kentucky Moms MATR Targeted Case Manager primary responsibility is that of working with pregnant women and 60 day postpartum mothers who have moderate to severe substance use dependence. This includes case finding, education, and assessing the person in an individual setting and making referrals to other helping agencies. Targeted Case Manager will coordinate relationships among other human services agencies, consult with courts, the Department for Community-Based Services (DCBS), domestic violence shelters, law enforcement, and the public in general. Targeted Case Manager will be the initial contact worker for referrals received from local health departments. Targeted Case Manager will continually assess the individual needs of their participants and reflect these needs in staff meetings involving various community partners, evaluations, treatment plans, and chart progress notes. This person will be able to demonstrate and consistently practice competency in the skills, knowledge and attitudes as outlined in Addiction Counseling Competencies: The Knowledge, Skills, And Attitudes of Professional Practice (CSAT TAP #21, 2006). Targeted Case Manager must demonstrate respect and non-judgmental attitudes toward participants in all contacts with community professionals and agencies. This position requires travel within our service region, home visits with participants, and attendance is required at quarterly meetings scheduled by the funding source.
Skills/Knowledge: Targeted Case Manager must successfully complete DBH approved curriculum of targeted case management training within six (6) months of employment and maintain continuing education hours for certification. Targeted Case Manager would benefit from working knowledge and understanding of domestic violence, sexual trauma, and human trafficking.
